About this project
The problem we solve: Every year, 19 million ambulances arrive to Emergency Departments (ED) in the United States. That's 60 ambulances every second. Yet across the country, paramedics and EDs still rely on radio transmissions over public channels filled with static and interference. Without a patient name, EKG, photo or video, diagnosis is often delayed until the patient arrives, leading to wasted time and costly delays in treatment. For the 1.5 million Americans who will suffer a stroke or heart attack this year, every moment counts.
In fact, according to the Joint Commission, failures in communication remain the number 1 cause of delays in treatment in Emergency Departments today. Delays in the treatment of heart attacks and strokes alone result in $2.1 billion in direct costs to the health system, from longer length of stays and higher readmission rates, while patients and our communities pay billions more in long-term morbidity and disability. About our solution: Twiage is a HIPAA-compliant, interoperable prehospital platform that delivers real-time data from ambulance to hospital. EMS providers use the Twiage smartphone app to share patient demographics, vital signs, EKGs, photos, and videos. Each case is tracked by GPS and sent to hospitals in real-time via the Twiage dashboard, a visual tracker of all incoming ambulances to the hospital. By providing EDs with rich situational awareness of their inbound traffic, Twiage enables hospitals to accurately predict patient arrival and prioritize resources. Moreover, Twiage data is available in real-time to all members of the care team and viewable on any phone, tablet, or computer. Compared to radio, Twiage is more secure, easier to use, and delivers earlier notifications by enabling on-scene data collection. For emergencies such as stroke, heart attack, and trauma, Twiage helps hospitals save crucial minutes on door-to-provider, door-to-CT, and door-to-needle times when it matters most. Progress to date: Twiage is live at two hospitals and 7 EMS agencies in Massachusetts and New Jersey. Today, Twiage services 911 medical emergencies for over 300,000 people in Eastern Massachusetts has helped accelerate care of critical emergencies, like stroke, saving an average of 5-8 minutes per patient case. Twiage has been profiled in Fast Company, the Washington Post, and the Boston Globe, and its role in improving prehospital care will be presented at an oral presentation at the American Telemedicine Association National Meeting in May 2016.
Our traction to date includes pilot commitments from 20 hospital health systems across the country, including some of the largest not-for-profit and for-profit hospital systems in the U.S. Twiage was named mHealth Rockstar of the Year for “developing the most innovative and meaningful mobile health technology” of 2014, and was named winner of Cleveland Clinic’s New Venture Healthcare Challenge in 2015.

A published researcher and 40 Under 40 Healthcare Innovator, YiDing is passionate about improving health care. During her residency, she founded Twiage to replace outdated radio systems used by ambulances and hospitals that contribute to delays in care. She is the recipient of numerous awards and her work has been featured in the Washington Post, Fast Company, and the Boston Globe.

Fortunately, today’s medical advances enable doctors to halt strokes and heart attacks and reverse their devastating effects if treated within a small window of time. First responders and ambulances play a critical part in accelerating life-saving care, but they rely on outdated radio transmissions to notify doctors at the hospital, leading to unnecessary delays in patient care.
Twiage is here to change that. Free for first responders and EMS in your community, Twiage helps medics collect vital information from the moment they are on the scene and share it with doctors at the hospital in real-time. Hospitals are able to pre-register patients, mobilize critical resources, and track every incoming ambulance to deliver the right care, for the right patient, at just the right time. Twiage accelerates life-saving care so that patients can live longer, healthier lives.
How We Help Physicians
There are 19 million ambulance rides to EDs every year. That’s 60 ambulances per second. For ED physicians and on-call teams, EMS notification and documentation can be sparse or even absent. Without a patient name, EKG, photo, or video, medical teams are often in a black box until the patient arrives. And yet for critical emergencies such as heart attack, stroke, sepsis, and trauma, every moment counts.
Twiage is a HIPAA-compliant, interoperable platform that shares real-time data between ambulances and hospitals. With live GPS-tracking and patient IDs, Twiage allows care teams to preregister patients, track patient arrival, and monitor clinical status en route. Doctors can receive real-time alerts, patient demographics, vital signs, photos, and videos on any smartphone, tablet, or computer. By providing rich, clinical data to the medical team, Twiage empowers doctors to mobilize the critical resources that accelerate patient care and improve patient outcomes.
Challenge Mission
Mission: As a PGY-2 resident, I founded Twiage to help improve prehospital care. Today, Twiage empowers other physicians to innovate and adapt this novel platform to improve patient care in their community. Use of funds: Twiage will use the award funds to support development of new features that will support mobile integrated healthcare (or community paramedicine). A comprehensive prehospital system, Twiage already supports routine, urgent, and emergent cases, but the new features will better reflect ambulatory care and care for patients at home. We believe that these features will help our customers (EMS providers and health care systems) deliver better patient care in the home, while reducing unnecessary hospitalizations and readmissions. Intellectual Property Status: Twiage, LLC, owns all of the intellectual property for this project. Twiage uses proprietary decision support and triage algorithms. Patent pending on its clinical smartphone app. FDA Status: Twiage stores and transmits electronic medical information and is exempt from FDA regulation. Personal Message: Today, a few words over the radio are the best a paramedic can give, and the best a doctor can receive about a critically ill patient arriving to the ED.
