About this project
The problem we solve: Chronic diseases are responsible for 70% of all deaths in the United States and 86% of all US healthcare spending – or $2.8 trillion annually. Today 133 million Americans live with at least one chronic disease with a projected 81 million Americans likely to be living with multiple chronic diseases by 2020. In addition to being the leading cause of death and disability in patients, chronic diseases add huge demand for healthcare. 81% of hospital admissions, 91% of all prescriptions filled, and 76% of all physician visits are accounted for by patients with chronic diseases. Additionally, chronic diseases also account for the vast majority of healthcare spending, with 99% of all Medicare and 83% of Medicaid spending going to diagnosing and treating chronic diseases. Addressing this problem requires complete restructuring and disruption of the current healthcare system. Incentives for such a transformation exist, but the infrastructure and technology is lacking. About our solution: We propose an app that seamlessly integrates to all EMRs to create an intuitive, simple to use user interface for providers that simplifies chronic care management and its reimbursement. Currently, although incentives for disrupting the model of healthcare exist, the infrastructure of much of healthcare remains stagnant. EMRs still mainly function as manual data recording devices, making management of chronic care conditions clunky and time consuming. Our app, built using the fhir API to seamlessly integrate with all EMRs, overlays the EMR and extracts relevant data, displaying it in an easy to read fashion to allow providers to make a quick, accurate assessment and adjust the care management plan as needed. Furthermore, our app addresses all reimbursement requirements for chronic care management, simplifying billing and reimbursement for chronic disease care. Finally, the app has a patient facing interface as well, increasing patient education and engagement. Progress to date: To date, discussions with local hospital systems and team members has begun to begin development on this app. A solution of this scale requires significant investment of time and money, yet has the potential to truly disrupt an industry and save countless lives. Cognizant of these facts, we are taking a lean approach, focusing on building, piloting, and iterating the product nearly simultaneously. Therefore, we are looking to build relationships with organizations and people interested in not only testing the app, but developing it as well.

How We Help Patients
About a third of all Americans live with at least one chronic disease. Chances are, you or someone you know suffers from one. Unfortunately, the current healthcare system is not built to best treat these diseases. We at ChroniCareHero understand that, and are working diligently to offer simple, intuitive tools to both patients and doctors to help in the care of chronic diseases. We are creating user friendly interfaces to help doctors manage chronic diseases, so that they are encouraged to talk and work with you on developing a chronic care plan. Our patient facing ChroniCareHero app works by displaying relevant parts of the health record in a user-friendly, readable interface. This app also works to display the personalized chronic care management plan (which can be adjusted by the patient or doctor), show progress in daily and weekly goals, integrate with wearable devices, and educate patients on their chronic conditions, creating healthier, happier patients!
How We Help Physicians
Although policymakers are slowly beginning to embrace new models of healthcare, physicians still mostly live in a system designed for reactionary, acute care management. Reimbursement schemes for chronic care management are clunky, time consuming, and complex, especially when using EMRs and organization systems not built for chronic disease care. This is where ChroniCareHero steps in, finally delivering a tool made by medical professionals for medical professionals to simplify and automate much of the frustrating work of data mining. ChroniCareHero uses an intuitive, actionable user interface to allow for better care and simpler reimbursement for care provided. ChroniCareHero easily integrates with all EMRs, simply requiring a download from the smart on fhir app store. Finally, ChroniCareHero uses a patient facing app to help physicians educate and engage patients in their own care, so doctors spend less time checking boxes, and more time caring for patients.
Challenge Mission
Mission: This medical student innovation focuses on usability and simplicity in offering solutions to physicians and patients to improve chronic disease care, and patient engagement. Use of funds: The funds would be used to pilot and develop the app and acquire early customers and users. Intellectual Property Status: The founder - Victor Sadauskas - is the sole owner of all IP involved with this project. FDA Status: None yet, plan to get FDA clearance in the future. Personal Message: About a third of all Americans suffer from chronic diseases with $2.8 trillion dollars being spent to treat them. Yet, our healthcare system is woefully outdated in addressing this issue. That’s where ChroniCareHero comes in. Focusing on simplicity, usability, and integrative capabilities, this app helps physicians get paid for the work they do in chronic care management, and engages and educates patients in their own care, to create a healthier, happier patients!
